HOW WILL LOVE FOCUS YOUTH OUTREACH THP-NMD HELP YOUTH?

  1. A less restricted safe place to live, which you can call home.
  2. You will be assisted all the way on your transition to self-sufficient and successful adulthood.
  3. You will be assisted in identifying your strengths and needs.
  4. You will be assisted in your preparation to live independently, thus reducing the likelihood for homelessness.
  5. You will be provided service planning to identify your goals and how to achieve them.
  6. You will be linked to available needed community resources.
  7. You will be supported in your search to connect with biological or non-related family members.
  8. Monitor your progress with employers and service providers.
  9. Advocate for your rights or your need to receive services.
  10. Coach you in “role plays” for situations with employers or service providers.
  11. You will receive monthly allowance for basic living needs.

Eligibility & Admission Requirements

Current foster youth ages 18 up to 21 in out-of-home placement with Child Welfare or Probation are eligible. For youth placed out of county, services are coordinated with the county where a youth is residing. To refer a youth, a Social Worker or Probation Officer is required to submit a referral form and the Transitional Independent Living Plan (TILP). Intake Application is online.

Independent Living Skills Classes

ILSC provides classes for life skills training such as employment, education, housing, and money management. Financial incentives are paid to youth who decide to participate in the Love Focus Youth Outreach THPP-NMD.
